Once the claim is identified the details of the claim is given which can include patient, provider, service type, date of service, deductible, amount paid, amount owed, and other information.

Find claim based on claim number, plan (medical, dental, vision), patient (self or family member), provider (doctor), service type(doctor visit, hospital), date of service, or a combination of information.
 
Give Details of Claim
Give details of the claim which can include patient, provider, service type, date of service, deductible, amount paid, amount owed, and other information.
